Accident and Incident Report Form

About this document

What it’s for

Practical guidance on keeping an accident book and deciding whether an incident must be reported under RIDDOR, followed by a detailed report and investigation form and a near-miss report.

When to use it

  • After any injury at work, however minor, to an employee, contractor, visitor or member of the public
  • For near misses, dangerous occurrences and work-related ill health
  • As the investigation record for more serious incidents and insurance claims

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • Section 1 – Guidance: accident book and record keeping
  • Key terms
  • Your accident book
  • Section 2 – RIDDOR: what must be reported, and when
  • Part A – Person affected
  • Part B – What happened
  • Part C – Injury or ill health
  • Part D – First aid and treatment
  • Part E – Witnesses
  • Person completing Parts A to E
  • Part F – Immediate action taken
  • Part G – RIDDOR and notifications
  • Part H – Investigation
  • Part I – Corrective and preventive actions
  • Part J – Sign-off

How to complete it

  • The injured person (or a colleague) completes Parts A to E as soon as possible
  • The manager completes Parts F to H, deciding whether RIDDOR applies
  • Investigate in proportion to the seriousness and record actions in Part I
  • Store completed forms securely – they contain personal and sometimes health data

Key points to check

  • RIDDOR time limits are short – some reports must be made without delay
  • Northern Ireland has its own RIDDOR rules with different thresholds – see Section 2
