Care Plan Template

About this document

What it’s for

A complete care and support plan for one person, written with them. It starts with a one-page profile and the person’s own outcomes, then sets out how staff should support them in 18 areas of daily life, with risks, consent, emergency information, agreement and a review log. It is designed to meet CQC Regulation 9 (person-centred care).

When to use it

  • After the needs assessment and before care starts (or within the first 48 hours of an urgent start)
  • For home care and care home services – delete areas that do not apply
  • As the working document staff follow, kept up to date at every review

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • Section 4 – My visit schedule or daily routine

How to complete it

  • Write it with the person and the people they want involved, using “I” and “my”
  • Complete Sections 1 to 4 first; then each relevant area in Section 5
  • Link each risk to a separate risk assessment where needed
  • Agree and sign it (Section 9) and record every review (Section 10)

Key points to check

  • Consent and capacity must be recorded for every significant decision
  • Review at least every [3–6] months and whenever needs change
