CIS Subcontractor Onboarding and Verification Form

About this document

What it’s for

A form for accountants, bookkeepers and payroll bureaus that run the Construction Industry Scheme for contractor clients. The subcontractor completes Parts B and H; the contractor and bureau complete the status, scope, verification and supply chain checks, so every payment starts at the right deduction rate and the client can show HMRC it took reasonable care.

When to use it

  • Before a contractor client pays a new subcontractor for construction work
  • When a subcontractor changes business structure (for example sole trader to company)
  • As an annual refresh for regular subcontractors, especially those with gross payment status

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • Part A – Contractor details
  • Part B – Subcontractor details (subcontractor to complete)
  • Part C – Employment status check
  • Part D – Is the work within CIS?
  • Part E – HMRC verification
  • Part F – Supply chain due diligence
  • Part G – Payment and return set-up
  • Part H – Subcontractor declaration
  • Part I – Approval to pay
  • Subcontractor Verification Log

How to complete it

  • Send Parts B and H to the subcontractor before work starts
  • Complete the status and scope checks in Parts C and D
  • Verify with HMRC and record the result exactly in Part E
  • Do not set up the first payment until Part I is signed

Key points to check

  • The April 2026 CIS changes: nil returns, supply chain fraud penalties and faster loss of gross payment status
  • Deduction rates and the verification rules – check HMRC guidance (CIS340) before use
