Client Approval and Sign-off Form

About this document

What it’s for

A form that captures the client’s decision at each stage of a design, photography, video or web project, the revision rounds used, any change requests and their cost, and final acceptance. It creates a clear audit trail if there is later a dispute about what was agreed, whether extra work was authorised, or whether an error was signed off.

When to use it

  • At each approval milestone (concept, first draft, final draft, final delivery)
  • Whenever the client asks for something outside the agreed scope
  • Before files go to print, broadcast, paid media or a live website

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  1. Approval
  2. Deemed approval
  3. Revision rounds

Also included

  • Part 1 – Project details
  • Part 2 – Approval stage plan
  • Part 3 – Stage approval
  • Items submitted for approval
  • Client decision (tick one)
  • Consolidated feedback
  • Client checks before approval
  • Part 4 – What approval means
  • Supplier assessment
  • Decision
  • Final deliverables
  • Outstanding items (snagging list)
  • Client confirmation

How to complete it

  • Complete Part 1 once at the start and the stage plan in Part 2
  • Send Part 3 with each draft, and Part 5 for each change request
  • Ask the client to sign Part 6 on final delivery
  • An email reply confirming the form is fine – save it with the project file

Key points to check

  • Make sure the person approving has authority to do so
  • Deemed approval only works if the client has agreed to it in your terms
  • Track revision rounds so extra rounds can be charged
