Client Consultation Form

A full consultation form to complete with every new client before their first treatment, and to review at later visits. It helps you spot contraindications, choose a safe treatment, meet your insurer’s requirements and keep a clear record if anything goes wrong. It includes the privacy wording you need because health information is “special category” data under UK GDPR.

• Before the first treatment for every new client, whatever the service
• At every later visit, using the review table at the end to record changes
• Before any new type of treatment, especially injectables, lasers, peels, needling or electrical treatments

• Add your business details and delete parts that do not apply to your services (for example Part G if you do not offer aesthetics)
• Go through the form with the client – do not just hand it over – and record your assessment in Part H
• Ask the client to read Part I and sign the declarations in Part J
• Store the completed form securely with the Client Record Card
