COSHH Assessment Form

About this document

What it’s for

A Control of Substances Hazardous to Health (COSHH) assessment pack: an inventory of the hazardous substances you use or produce, and a detailed assessment form for each substance or process covering hazards, exposure, controls, storage, emergencies, health surveillance and review.

When to use it

  • For any product with a hazard pictogram or safety data sheet – cleaning chemicals, adhesives, paints, solvents, fuels
  • For substances produced by work – dusts (wood, silica, flour), fumes (welding, soldering), mists, biological agents
  • Before introducing a new substance or process

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • Part 1 – Hazardous substances inventory
  • A. Substance details
  • B. Hazard classification (tick all pictograms on the label or SDS)
  • C. The task and exposure
  • D. Control measures
  • E. Storage, spills, emergencies and disposal
  • F. Monitoring and health surveillance
  • G. Information, instruction and training
  • H. Conclusion

How to complete it

  • List every substance in the inventory first, then assess each one (or group very similar ones)
  • Use Section 2, 7 and 8 of the safety data sheet for hazards, handling and exposure controls
  • Watch how the substance is actually used before deciding on controls
  • Share the controls with every user and keep the form with the safety data sheet

Key points to check

  • Asbestos, lead and radioactive substances have their own regulations and are outside this form
  • Northern Ireland: the Control of Substances Hazardous to Health Regulations (Northern Ireland) 2003 apply
