Customer Credit Application Form

About this document

What it’s for

A form for a business customer to complete when applying for a credit account with you. It collects everything you need to check the applicant, decide a credit limit and chase payment efficiently: legal identity, ownership, trading history, finance contacts, bank details, trade references and credit requirements.

It also gets the applicant to sign up to your terms of trade, consent to credit checks, and (optionally) give a personal guarantee from a director, partner or owner. A clear data protection notice is included because credit checks involve personal data about individuals.

When to use it

  • Before supplying any new customer on credit
  • When an existing customer asks for a significant increase in its credit limit
  • When a customer’s ownership or legal structure changes (for example a sole trader incorporates)

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • How to apply
  • Section 1 – Business details
  • Section 2 – Owners, directors and partners
  • Section 3 – Contacts
  • Section 4 – Bank details
  • Section 5 – Credit requested
  • Section 6 – Trade references
  • Section 7 – Financial information
  • Section 8 – Security and credit insurance
  • Section 9 – Personal guarantee [OPTIONAL]
  • Section 10 – Declaration and acceptance of terms
  • Section 11 – How we use your information (data protection notice)
  • Section 12 – Consent to personal credit searches (sole traders, partners and guarantors)
  • For office use only

How to complete it

  • Add your business details, your standard terms and the credit agencies you use
  • Decide whether to include Section 9 (personal guarantee) – delete it if not needed
  • Make sure the data protection notice in Section 11 matches your privacy notice and names your credit reference agency
  • Complete the “Office use” section when you approve the account, and keep the signed form on file

Key points to check

  • Your terms and conditions of sale should be attached or clearly referred to, so they become part of the contract
  • A personal guarantee is a serious commitment – the guarantor should be told to take independent advice, and must sign personally
  • Only ask for personal information you genuinely need, and keep it securely
