Display Screen Equipment Assessment

About this document

What it’s for

A display screen equipment (DSE) workstation assessment covering the screen, keyboard, mouse, laptop, chair, desk, environment, software, work routine and health. The user completes the checklist and an assessor reviews it and agrees actions.

When to use it

  • For every “user” – someone who normally uses DSE daily for continuous periods of an hour or more
  • When a user starts, moves desk, gets new equipment, or starts working from home
  • When someone reports discomfort, and at regular reviews (for example every two years)

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • Part 1 – User details
  • Part 2 – Workstation checklist
  • A. Display screen
  • B. Keyboard
  • C. Mouse and other input devices
  • D. Laptops and tablets
  • E. Software and work
  • F. Chair
  • G. Desk and space
  • H. Environment
  • I. Work routine, breaks and health
  • J. Homeworking (complete if you work at home)
  • Part 4 – Assessor’s findings and action plan
  • Part 5 – Eye and eyesight test record
  • Part 6 – Sign-off and review

How to complete it

  • Give the form to the user with time to sit at their workstation while completing it
  • Every “No” answer needs a comment and an action in the action plan
  • The assessor checks the answers, ideally by looking at the workstation (in person or by video/photo for homeworkers)
  • Keep the signed form and review it by the date shown

Key points to check

  • The law applies to home and hybrid workers who are users, as well as office staff
  • Northern Ireland: the Health and Safety (Display Screen Equipment) Regulations (Northern Ireland) 1992 are equivalent
