Fire Risk Assessment and Fire Safety Checklist

About this document

What it’s for

A complete fire risk assessment record for small and medium non-domestic premises (offices, shops, workshops, small warehouses, surgeries and similar). It covers fire hazards, people at risk, fire precautions, management arrangements, an overall risk rating, an action plan and an emergency plan.

When to use it

  • For premises you control as an employer, owner, occupier or managing agent (the “responsible person”)
  • When you first occupy premises, and at regular reviews
  • After alterations, a change of use, a fire or near miss, or a big change in occupancy

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • Part 1 – Premises and assessment details
  • Part 2 – Fire hazards
  • Sources of ignition
  • Sources of fuel
  • Sources of oxygen
  • Part 3 – People at risk
  • Means of escape
  • Fire detection and warning
  • Emergency lighting and signage
  • Firefighting equipment
  • Fire doors, compartmentation and fire spread
  • Part 5 – Fire safety management
  • Arrangements, training and records
  • If your building contains flats or other domestic premises (England)
  • Part 6 – Overall risk rating
  • Fire safety action plan
  • Part 7 – Testing and maintenance schedule
  • If you discover a fire
  • When you hear the alarm
  • Fire marshals

How to complete it

  • Work through Parts 1 to 6 on a walk-round of the whole premises, including roof spaces and plant rooms you can safely access
  • Answer every checklist item; anything marked “No” needs an entry in the action plan
  • Record the name of the person who carried out the assessment – this is now a legal requirement
  • Keep the completed record, test logs and certificates together

Key points to check

  • Complex, high-risk, sleeping or multi-occupied residential premises need a specialist fire risk assessor
  • Scotland and Northern Ireland have separate fire safety laws – see the first guidance note in the assessment
