General Risk Assessment

About this document

What it’s for

A general workplace risk assessment form that walks you through HSE’s five steps: identify the hazards, decide who might be harmed and how, evaluate the risks and decide on controls, record and implement your findings, and review.

When to use it

  • For each workplace, team or main activity – one form per area or activity is usually clearest
  • Before new work, equipment, premises or processes are introduced
  • To review an existing assessment after an accident, near miss or change

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • Assessment details
  • Step 1 – Identify the hazards
  • Physical and safety hazards
  • Health hazards
  • Organisational and people hazards
  • Step 2 – Decide who might be harmed and how
  • Step 3 – Evaluate the risks and decide on precautions
  • Likelihood and severity scales
  • Risk matrix (likelihood × severity)
  • Risk assessment table
  • Communicating the findings
  • Step 5 – Review the assessment and update if necessary
  • Sign-off

How to complete it

  • Walk round the workplace and talk to the people who do the work
  • Use the hazard prompt list in Step 1, then record each significant hazard in the Step 3 table
  • Score risk before and after your planned controls using the matrix
  • Turn every “further action” into a dated, owned item in the Step 4 action plan

Key points to check

  • Employers with five or more employees must record the significant findings – recording is good practice for everyone
  • Use the specific forms in this pack for fire, COSHH, manual handling, DSE and lone working
