Incident and Accident Report for Care Services

About this document

What it’s for

A report form for any accident, incident or near miss involving people who use the service, staff or visitors. It captures the facts, injuries and immediate action, then guides the manager through every external notification that may be required, the duty of candour check, investigation and learning.

When to use it

  • After any fall, injury, medication error, missed visit, near miss, aggression, property damage or other incident
  • When deciding whether to notify CQC, the local authority, the police, HSE or others
  • As the basis of your incident log and trend analysis (Regulation 17)

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • Part 1 – Incident details
  • Part 2 – People involved
  • Part 3 – Type of incident (tick all that apply)
  • Part 4 – What happened
  • Part 5 – Injury and harm
  • Body map (describe location: front / back, left / right)
  • Part 6 – Immediate action
  • Part 7 – Witnesses and reporter’s declaration
  • Part 8 – Notifications checklist (manager)
  • Part 9 – Manager’s review and investigation
  • Action plan
  • Part 10 – Duty of candour check
  • Part 11 – Learning and sign-off
  • Appendix B – After a fall: checklist

How to complete it

  • Staff complete Parts 1 to 7 as soon as possible after the incident, before the end of their shift
  • The manager completes Parts 8 to 11 and decides on notifications
  • Use the quick guide in Appendix A to check CQC notification triggers
  • File the report securely and record it in the incident log

Key points to check

  • CQC notifications must be made “without delay” – do not wait for an investigation to finish
  • Check the current CQC notification forms and online service before use
