Patch Test Record

A record of each patch test (skin allergy test) you carry out before colour, tint, lash and other chemical services, and its result. Insurers usually require a patch test where the manufacturer’s instructions call for one, and a dated, signed record is your best evidence if a client later has a reaction.

• Before a client’s first colour, tint, lash lift, lash extension or similar service
• Before each later service where the manufacturer’s instructions say so, or under your repeat policy
• Whenever the product, health, medicines or pregnancy status changes

• Complete Parts A to C when you apply the test and give the client the instruction slip in Part D
• Check and record the result in Part E before the service – never earlier than the time the manufacturer specifies
• Use the log in Part G to track tests across all clients
