Payroll Changes Form

About this document

What it’s for

A period-by-period instruction form that an employer client sends to its payroll bureau (or a manager sends to in-house payroll) listing every change to be processed, with the evidence and authority payroll needs. It reduces errors, missed deadlines and arguments about what was instructed.

When to use it

  • Every pay period, before the payroll cut-off date
  • Whenever an employee is sick, starts family leave, joins a salary sacrifice scheme or leaves
  • When a court order, council or government agency asks you to deduct from an employee’s pay

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • Part A – Submission details
  • Part B – Permanent pay and hours changes
  • Part C – One-off payments and deductions this period
  • Part D – Sickness absence
  • Part E – Family-related leave and statutory payments
  • Part F – Salary sacrifice and other voluntary deductions
  • Part G – Attachment of earnings and deduction orders
  • Part H – Leavers
  • Part I – Other changes
  • Part J – Authorisation
  • Part K – Payroll bureau use

How to complete it

  • Fill in only the sections that apply this period; write “none” in the others
  • Attach the evidence listed (fit notes, MATB1, orders, signed agreements)
  • An authorised signatory signs Part J
  • Payroll records receipt and processing in Part K

Key points to check

  • Statutory payment rates and thresholds change each April – check current figures
  • Some changes need the employee’s written agreement before they are processed
