Permit to Work Forms

About this document

What it’s for

A permit to work is a formal written system used to control high-risk work. It makes sure the hazards have been thought about, the precautions are actually in place before work starts, the people doing the work know the limits, and the area is handed back safely. This pack contains a short permit procedure, a register and four permits.

When to use it

  • Hot works – welding, cutting, grinding, brazing, torch-on roofing
  • Entry into confined spaces – chambers, tanks, sewers, voids
  • Higher-risk work at height – roofs, fragile surfaces, use of harnesses
  • Work on electrical systems that must be isolated and proved dead

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • 1. Purpose
  • 2. Work that needs a permit
  • 3. Roles
  • 4. Rules
  • 5. Permit register
  • Section 1 – Work to be done
  • Section 2 – Precautions (tick each one when checked in place)
  • Section 3 – Authorisation
  • Section 4 – Acceptance by permit holder and team
  • Section 5 – Extension (if allowed)
  • Section 6 – Completion and handback
  • Section 2 – Isolation details

How to complete it

  • Nominate the authorised persons who may issue permits
  • Issue a permit for each task and shift and give it a unique number
  • Check the precautions physically before signing – do not tick from the office
  • Close and file the permit and update the register when the work is finished
