Person-Centred Needs Assessment

About this document

What it’s for

The provider’s own assessment of a person before their care starts (a pre-admission or initial assessment). It gathers what you need to decide safely whether you can meet the person’s needs, to agree the start, and to write the care plan. It covers 20 areas, a home environment check, the Care Act eligibility outcomes and a sign-off decision.

When to use it

  • Before accepting a new person into home care or a care home
  • When a person returns from hospital with significantly changed needs
  • Alongside, not instead of, any local authority or NHS assessment

How to complete it

  • Do it face to face where possible, with the person and anyone they want present
  • Obtain the commissioner’s assessment and hospital discharge information first
  • Complete the parts that apply and record the person’s own words
  • Complete Part 22 to record whether you can meet the needs, then build the care plan

Key points to check

  • Only accept a person if you have the skills, staff and equipment to meet their needs (Regulation 12)
  • Use validated tools (e.g. falls, nutrition, pressure risk) as your clinical leads advise
