Portfolio and Credit Permission Form

About this document

What it’s for

A short agreement in which the client chooses how, when and where you may show the work you created for them, how each of you will be credited, and what must stay confidential. It avoids awkward takedown requests and protects your ability to market your business.

When to use it

  • At the start of a project, or on final delivery
  • When a client has signed an NDA or the work is confidential or unreleased
  • Before entering work into awards, publishing a case study or using a client’s logo or testimonial

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  1. Scope of this permission
  2. Changes and withdrawal
  3. Data protection
  4. General

Also included

  • Section 1 – Parties and work covered
  • Section 2 – Permission level (choose one)
  • Section 3 – Channels (tick all that apply)
  • Section 4 – Timing
  • Section 5 – Confidentiality options (tick all that apply)
  • Section 6 – Credits, logos, testimonials and awards
  • How the Creator will be credited by the Client
  • How the Client will be described by the Creator
  • Awards and press
  • Section 7 – People and third-party material
  • Section 8 – NDA and confidentiality carve-out
  • Section 9 – Terms

How to complete it

  • List the work covered in Section 1
  • Ask the client to choose one permission level in Section 2 and tick the options that apply in Sections 3 to 6
  • If an NDA applies, add the carve-out wording in Section 8 to it
  • Sign, and keep a copy with the project records

Key points to check

  • Permission to show the work is not a licence for anyone else to use it
  • People shown in the work must have consented to portfolio use too
  • Respect takedown requests promptly to protect the relationship
