Safeguarding Concern Report Form

About this document

What it’s for

The form staff, volunteers and trustees complete when they are worried about a child or adult at risk, when someone tells them about abuse, or when there is a concern about one of your own people. The second half records what the Designated Safeguarding Lead (DSL) decided and did – the audit trail you need if a decision is ever questioned.

When to use it

  • Every time someone has a safeguarding concern, however minor
  • After a disclosure – complete it the same day
  • For low-level concerns about the behaviour of staff, volunteers or trustees

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • Before you start
  • Part 1 – About you (the person raising the concern)
  • Part 2 – About the person you are concerned about
  • Part 3 – What kind of concern is this?
  • Part 4 – What happened?
  • Part 5 – Concern about a member of staff, volunteer or trustee
  • Part 6 – What you have done so far
  • Part 7 – Receipt and initial assessment
  • Part 8 – Decision and referrals
  • Part 9 – Reporting to regulators, insurers and funders
  • Part 10 – Follow-up, outcome and closure

How to complete it

  • Add your DSL’s name and number in the box at the top before you print copies
  • Keep blank copies wherever activities take place and in a shared (restricted) electronic folder
  • The person raising the concern completes Parts 1 to 6 and hands the form to the DSL the same day
  • Only the DSL or deputy completes Parts 7 to 10

Key points to check

  • Completed forms contain sensitive personal data – store them securely and separately from general files
  • Make sure the form matches your Safeguarding Policy and local procedures
