Site Health and Safety Plan (Construction Phase Plan)

About this document

What it’s for

Every construction project in Great Britain needs a construction phase plan before work starts on site (regulation 12 of the Construction (Design and Management) Regulations 2015). This template gives small builders and trades a complete, proportionate plan: project details, who does what, site rules, the significant risks and how you will control them, welfare, emergencies and monitoring.

When to use it

  • Any building, refurbishment, extension, repair, maintenance or demolition job – domestic or commercial
  • When you are the only contractor (you draw up the plan) or the principal contractor (you draw it up with input from the principal designer)
  • Before any work starts on site, then keep it up to date as the job changes

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • Part 1 – Project details
  • Part 2 – Dutyholders and key contacts
  • Subcontractors and trades
  • Part 3 – Management arrangements
  • 3.1 Roles and responsibilities on site
  • 3.2 Planning and coordinating the work
  • 3.3 Competence, induction and training
  • 3.4 Consultation with workers
  • 3.5 Communication and information
  • Part 4 – Site rules
  • Part 5 – Existing information and site hazards
  • Part 6 – Significant risks and how they will be controlled
  • Part 7 – Welfare facilities
  • Part 8 – Emergency arrangements
  • Accident reporting
  • Part 9 – Health and safety file and handover
  • Part 10 – Monitoring and review
  • Part 11 – Agreement to the plan

How to complete it

  • Fill in the project details and contacts in Parts 1 and 2
  • Delete risks in Part 6 that do not apply and add any that do – keep it specific to this site
  • Brief everyone on site on the plan and keep a copy on site
  • Review it whenever the work, the team or the risks change

Key points to check

  • Whether the project must be notified to HSE (form F10) – see Part 1
  • Whether there is more than one contractor, which means a principal designer and principal contractor must be appointed
