Source of Funds and Source of Wealth Declaration and Evidence Record

About this document

What it’s for

A two-part form. Part A is completed and signed by the client and explains the source of the funds involved in the relationship or a transaction and, where needed, the source of the client’s overall wealth. Part B is your record of the evidence obtained to corroborate it and your conclusion. It supports enhanced due diligence under regulations 33 to 35 and the CCAB 2026 guidance on when to ask about source of funds.

When to use it

  • Every politically exposed person, family member or close associate (regulation 35 requires source of wealth and source of funds)
  • Every high-risk client and any transaction that is unusually large or unusually complex
  • Whenever activity seems inconsistent with what you know of the client, its business or risk profile

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • Part A – Client declaration
  • Part B – Firm evidence record

How to complete it

  • Send Part A to the client with a short covering note – the wording is neutral and does not suggest suspicion
  • Corroborate the declaration with independent evidence proportionate to the risk and record it in Part B
  • Have the MLRO review the conclusion for every PEP and high-risk client

Key points to check

  • Updated for the Money Laundering Regulations as amended by SI 2026/621 and the CCAB 2026 guidance
  • Tailor it to your firm before use
