Staff Supervision Record

About this document

What it’s for

A complete set of forms for formal supervision of care workers: a supervision agreement, a detailed record covering wellbeing, practice, the people supported, safeguarding, training and actions, an observation of practice (spot check) form and a planner. Together they evidence the support, supervision and appraisal staff must receive under Regulation 18.

When to use it

  • For planned one-to-one supervision, at the frequency in your supervision policy
  • For spot checks of home care visits or observed practice in a care home
  • To prepare for appraisals and to show CQC how staff are supported

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • Part 1 – Supervision agreement
  • What we agree
  • 1. Review of actions from last session
  • 2. Wellbeing and workload
  • 3. The people supported
  • 4. Practice and quality of care
  • 5. Safeguarding, incidents and reflective practice
  • 6. Policy and knowledge check
  • 7. Training and development
  • 8. Any other business
  • 9. Agreed actions
  • 10. Sign-off

How to complete it

  • Agree and sign the supervision agreement at the first session
  • Send the record template to the worker in advance so they can prepare
  • Complete every section together, agree SMART actions and both sign
  • Keep records confidential on the personnel file and track dates on the planner

Key points to check

  • Anything that suggests risk to a person supported must be acted on at once, not held for the next session
  • Supervision is not a disciplinary meeting – keep conduct processes separate
