Statement of Purpose

About this document

What it’s for

Every CQC-registered provider must have a statement of purpose containing the information in Schedule 3 to the Care Quality Commission (Registration) Regulations 2009, keep it under review and tell CQC about changes. This template covers the five required items for each location, plus optional sections that make it useful to people choosing a service.

When to use it

  • When applying to register with CQC, or to add a location or regulated activity
  • Whenever your aims, services, people supported, manager or locations change
  • As part of the information you give people before they choose your service
