Supplier Comparison and Selection Form

About this document

What it’s for

A structured way to choose between suppliers of goods or services. It separates deal-breakers (pass or fail) from preferences (weighted scores), so you pick the supplier that gives the best overall value rather than simply the cheapest quote, and it leaves a written record of why you chose them.

When to use it

  • Choosing a key supplier of stock, materials, packaging, software, IT, premises services or outsourced work
  • Re-tendering a contract or reviewing an under-performing supplier
  • Any purchase where you would want to explain your choice to a partner, funder or investor later

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • Part A – What we are buying
  • Part B – Must-have requirements (pass / fail)
  • Part C – Scoring criteria and weights
  • Scoring scale
  • Scoring notes (evidence for each score)
  • Worked example
  • Part F – Decision record
  • Sign-off

How to complete it

  • Write down your requirement in Part A before you ask for quotes
  • Agree the must-haves (Part B) and criteria weights (Part C) before you score anyone
  • Score each supplier 0–5 against each criterion and work out the weighted scores in Part D
  • Carry out the checks in Part E on the preferred supplier, then record the decision in Part F

Key points to check

  • Weights must add up to 100
  • Compare like with like: total annual cost, not unit price alone
