Supplier Set-up Form

About this document

What it’s for

A supplier onboarding form that collects everything you need before paying a new supplier: legal identity, VAT and tax status, bank details, insurance, compliance information and your internal due diligence checks. It includes a mandatory telephone call-back procedure and a separate form for changes to bank details – the point at which most invoice fraud happens.

When to use it

  • Before setting up any new supplier in your accounting or payment system
  • Whenever a supplier asks to change its bank details, name or address
  • When refreshing records for existing suppliers (for example annually for key suppliers)

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • Part 1 – Supplier details
  • Part 2 – Contacts
  • Part 3 – Tax and VAT
  • Part 4 – Bank details
  • Part 5 – Payment terms
  • Part 6 – Insurance and compliance
  • Supplier declaration
  • Internal use only – do not send to supplier
  • Part 7 – Request and due diligence checks
  • Part 8 – Bank detail verification (call-back)
  • Part 9 – Approval and set-up
  • Supplier change of details form

How to complete it

  • Send Parts 1 to 6 to the supplier to complete and sign, with evidence of their bank details
  • Your team completes Parts 7 to 9: checks, call-back verification and approval
  • Do not pay the supplier until every internal box is complete and approved
  • Keep the completed form with the supplier record

Key points to check

  • Never verify bank details using a phone number or email address given in the same request
  • Use the bank’s Confirmation of Payee result as an extra check, not a replacement for the call-back
  • Check construction subcontractors with HMRC under the Construction Industry Scheme before paying
