Treatment Consent Form

About this document

What it’s for

A written record that the client understood what the treatment involves, its likely results, its risks and alternatives, and freely agreed to go ahead. It includes ready-written risk modules for common treatments, separate photograph consent and a section on cooling-off and statutory cancellation rights.

When to use it

  • Before the first treatment of each type, after the Client Consultation Form
  • For every injectable or prescription-only treatment, every time
  • When the treatment, product or settings change significantly

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  • Part A – Client and treatment details
  • Part B – Age and eligibility
  • Part C – About your treatment
  • Part D – Risks and side effects
  • General risks – all treatments
  • Module 1 – Waxing, sugaring and threading
  • Module 2 – Lash and brow tint, lash lift, brow lamination
  • Module 3 – Lash extensions
  • Module 4 – Gel, acrylic and other nail enhancements
  • Module 5 – Facials, superficial chemical peels and microdermabrasion
  • Module 6 – Laser and IPL hair removal or skin treatments
  • Module 7 – Microneedling
  • Module 8 – Semi-permanent make-up and microblading
  • Module 9 – Botulinum toxin (prescription-only medicine)
  • Module 10 – Dermal filler
  • Module [11] – [OTHER TREATMENT]
  • Part E – Before your treatment
  • Part F – Aftercare and follow-up
  • Part G – Photographs
  • Part H – Time to think and cancellation rights

How to complete it

  • Keep Parts A to C and J for all treatments, then keep only the risk modules in Part D that apply and delete the rest
  • Talk the client through each risk and ask them to initial it
  • Complete the photo choices in Part G – never assume consent to marketing
  • Give the client a copy and the matching aftercare sheet

Key points to check

  • Age limits in Part B and current licensing rules for your area
  • Your prices, deposit and refund terms match your booking policy
