Under-18s Treatment Policy and Parental Consent Form

About this document

What it’s for

A policy setting out the legal age restrictions on salon and clinic treatments across the UK (sunbeds, botulinum toxin and fillers, tattooing, intimate piercing and others), your own age rules, how to check age and parental consent, and how to safeguard young clients. It includes a parental consent form and refusal record.

When to use it

  • If you treat, or might be asked to treat, anyone under 18
  • When training staff on age checks and safeguarding
  • When a parent asks you to treat their child
