Accident, Incident and Existing Injury Forms

About this document

What it’s for

A working set of forms that helps staff record accidents and injuries consistently, tell parents the same day, decide when Ofsted, the local child protection agencies or the Health and Safety Executive must be told, and spot patterns. It includes a head injury advice slip and a monthly log.

When to use it

  • Every time a child has an accident or injury in your care, however minor
  • When a child arrives with an injury that happened elsewhere
  • For incidents without injury (near misses, bites, property damage, intruders, evacuations)

What’s inside

The document is laid out in the sections below. Each one comes with a plain-English guidance note explaining what it does and what to change.

  1. Responding to an accident
  2. Telling parents
  3. Injuries a child arrives with
  4. Incidents without injury
  5. Notifications
  6. Review and learning

Also included

  • Notifications (manager to complete)

How to complete it

  • Print or set up digital copies of each form for every room
  • Complete the form as soon as possible and in any event before the child goes home
  • Have the parent sign to confirm they have been told; give them a copy
  • Review the monthly log for trends and act on them

Key points to check

  • An existing injury that worries you is a safeguarding concern – tell the DSL at once
  • Serious injuries must be notified to Ofsted within 14 days [CHECK CURRENT EYFS]
