Allergy and Dietary Requirements Form

A parent questionnaire and allergy care plan that records a child’s allergies, intolerances, medical diets, cultural and religious requirements, preferences and feeding or weaning stage. It also includes a safer eating checklist, a staff briefing record and a kitchen and room allergen card.

• Before every child starts – the EYFS requires you to obtain this information before admission
• Whenever a child’s diet, allergy or feeding stage changes
• At review, at least every [6] months for children with allergies

• Parents complete Parts 1 to 4; the manager completes Part 5 with them
• Attach the medical allergy action plan where the child has one
• Brief all staff, the cook and room staff and record it in Part 6
• Display the allergen card discreetly in the kitchen and room
