Annual Performance and Development Review (PDR) Form

About this document

What it’s for

The main annual appraisal form. The employee completes a self-assessment, the manager assesses objectives and behaviours against defined ratings, and together they agree development, wellbeing support and next steps, with second-line sign-off.

When to use it

  • For the end-of-year review under the Performance and Development Review Policy
  • For a review when someone changes role or leaves part-way through the year
  • Not for probation reviews or formal capability meetings – use those documents instead

  • Manager completes Parts 3 to 6 after the meeting; employee adds comments in Part 7
  • Both sign, the second-line manager countersigns and the employee gets a copy

Key points to check

  • Replace the sample values with your own (Part 3B)
  • Take account of disability, pregnancy and family leave before rating
